Our Fee policy
- Mixed Billing Practice – Effective from July 2022
- Concession cards include the following – Children under 16 with Medicare cards/Pensioner/DVA/Health Care Card
- Fee may vary depending on consult length or type
- Any wound reviews will be charged a “out of pocket” fee.
- General Consultation Fees

- Cancellations & Nonattendance – Failure to attend an appointment and late cancellation would occur a late cancellation fee of $40. (Minimum cancellation time frame is 3 hours prior to the appointment)
- Patients with No Medicare Card
- Patients without a valid Medicare or concession card will pay a private fee
- Q-fever vaccine & Employment Medicals – Please contact the clinic for quotation by calling 03 5572 5592 or email manager@hamiltonfamilypractice.com.au
- All the services carried out by the practice may charge a “out of pocket” fee, please enquire with the reception prior to booking.

APPOINTMENTS
If you require a longer appointment, please inform the reception so they could accommodate you at the time of booking.
If you required to get your paperwork done, please fill out the below form (HR/FO/RM/05) and bring it with you or email a day prior
to your appointment.
After Hours & Emergency Care
For all urgent emergencies after hours & whilst our Medical Centre is closed,

COMMUNICATION
Doctors & Nurses are available during normal surgery hours for emergency advice. Our Medical Staff are experienced in deciding the appropriate response to any phone request. Non-urgent phone messages will be returned at a time that is allocated for the Doctors to do so.
RECALL/REMINDERS
A computerised recall/reminder system is available and used for follow up of any medical conditions. If you do not wish to participate in this please inform your doctor. This surgery participates in State & National Registers.
It’s important that you make us aware if your contact details change, that way you will always receive these reminders/recalls.
TEST RESULTS
Our Doctors check results daily. Results will not be given out over the phone. Please make a follow up appointment at reception to discuss all results. RESULTS WILL NOT BE GIVEN OUT OVER THE PHONE OR BY EMAIL.

Please advise our Administration team or your doctor of any cultural background when you arrive for your appointment. This information assists us with preventative health outcomes.
EMERGENCY CONTACT DETAILS
If you need to update your personal details please see an Administration team member or your doctor when you arrive for your appointment. This way we can always get in contact with you or your next of kin/emergency contact if necessary.
REFERRALS
Doctors in this practice are competent at handling common health problems. When necessary, you may be referred to a specialist for further investigations and opinions. You can discuss this openly with your doctor.
Please be aware that there may be out of pocket fees for your specialist.
